District Nurse Team Leader will have responsibility and accountability for a Caseload of Patients and staff within a Cluster area.
The District Nurse Team Leader will be required to provide professional nursing care and support to patients, carers and families in the own home setting. Provide support and leadership to all staff. They will be responsible for ensuring a safe service delivery, which will include the development of the staff providing education and training.
Main duties of the job
The member of staff will be required to function as a member of the multi-disciplinary team, assess individual patient's care needs and subsequently develop, implement and evaluate appropriate plans of care.
The role will also include the supervision and instruction of qualified and unqualified members of the team.
The member of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required.
The member of staff will be required to be an active member of the Cluster
The ability to speak Welsh is desirable for this post English and/or Welshspeakers are equally welcome to apply.

- Specialist Practitioner Qualification, District Nursing
- Nurse prescribing Qualification
- Masters Qualification or willing to work towards
