We are looking for a motivated and committed nurse who wants to be a part of the Powys Tissue Viability team. We aim to deliver excellent standards of care to all people in our community. This can only be achieved by nurses who are committed to continuous professional development.

This is an ideal opportunity for someone able to demonstrate prior knowledge of tissue viability through their current roles and responsibilities. This role will aim to broaden their range of skills and knowledge whilst caring for people of all ages in their own environment or hospital as part of a multi-disciplinary team.

Main duties of the job

The Tissue Viability Specialist nurse will play a lead role to improve the care of patients with problems relating to but not limited to wound healing, potential for or reduced skin integrity. Communicate complex information to patients in relation to wound management, which can be extremely emotive when managing patients with other complex needs.

Working as part of the Powys Tissue Viability Specialist nurse team you will cover the South Powys catchment in both community and hospital settings. Base to be arranged according to successful applicant's requirements.

Working autonomously but also in partnership with the multi-disciplinary team to undertake assessments of complex cases who may also present with complex health needs that would potentially impact upon the patients' skin integrity or wound healing.

This post includes lone working and the ability to travel independently is essential. This is a part time post, hours are flexible.

The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.

Person Specification

Qualifications and/or Knowledge

Essential

  • Live registration with the NMC as Registered Nurse
  • Degree in Nursing or health related subject or equivalent experience
  • Experience as a link nurse to Tissue Viability / Dermatology/ Leg ulcer management or relevant clinical area


Desirable

  • Community Nursing Prescribing Qualification
  • Wound care qualification


Experience

Essential

  • Ability to assess and implement a range of evidenced based therapeutic care interventions
  • Proficient in multi-agency partnership working
  • Demonstrate autonomous working and management of own caseload
  • Evidence of continuing professional development
  • High level of knowledge of NMC code of conduct and professional standards/guidance
  • Proven ability to work under time constraints


Desirable

  • Delivery of clinical training in relation to specialism
  • Experience of proactive caseload management of wide range of patients


Aptitude and Abilities

Essential

  • Communicate effectively with multi-agency colleagues at all levels including negotiation, deescalation, conflict skills
  • Excellent report writing and presentation skills adopting local process
  • Proficient in dealing with highly complex and emotive issues
