JOB PURPOSE
The post holder will work autonomously and in collaboration with Acute Trusts, the Adult
Community Nursing Team, other health care professionals and statutory and voluntary agencies
to deliver excellent standards of patient care.
The post holder will be required to:
Work on a rotational shift basis within the Brent Rapid Response Team - service operating
hours 8am - 10:30 pm 7 day a week.
Be the Team Leader for designated shifts on a rotational basis responsible for day-to-day
management of the service.
Take designated responsibility for discrete areas of operational management/ clinical practice.
Prioritise and manage own workload with minimal supervision.
Main duties of the job
Responsible foro The post-holder will have clinical responsibility for assessment and treatment planning for clients referred to Maximising Independence. They will be expected to contribute to the development and monitoring of the service. Post holder may be asked to supervise rehab support workers and would be expected to supervise Occupational Therapy students on practice education placements.o The Maximising Independence team aims to provide a high quality, responsive rehabilitation service helping adults to regain their independence following illness or injury, to support them with their Long Term conditions and to promote health and wellbeingo The team comprises of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Rehabilitation Support workers and admin Assistants.o The service is delivered 7 days per week including bank holidays and there is an expectation that full time hours will be completed across 7 days as required.
