We are looking for a Consultant Liaison Psychiatrist(s) to work at The Royal Marsden Hospital. We have 12 PAs to recruit to and so we can employ one full time person or two part time staff with overlap to facilitate working relationships.

Main duties of the job

The post holder will be expected to provide:Clinical leadership of the general liaison psychiatry service to The Royal Marsden.Undertake direct clinical care of patients referred to the Adult Psychological Support Service with complex physical and psychiatric co-morbidities related to cancer diagnosis or impacting on cancer treatment and patient's quality of life .Provide clinical supervision to other team members working at Level 3 (NICE, 2004).Provide line management to other team members.Contribute to the weekly multidisciplinary APSS meeting, with particular responsibility for risk and medical issues.Integrated working with the existing Consultants (Medical and ClinicalOncologists and Surgeons) in The Royal Marsden Trust.Work with the patient's responsible clinical teams to assist in the assessment and management of people with suspected or identified mental health needs and helping to manage their clinical needs.Provide expertise and advice to support the appropriate use of the Mental Capacity and Mental Health Act including, where appropriate, completion of assessments to support clinical and legal decision making .Acting as Responsible Clinician and exercising section 12 powers in respect of the Mental Health Act for inpatients.Take part in or facilitate investigations and debriefing following seriousuntoward incidents related to patients with mental health comorbidities.

Job responsibilities

The Adult Psychological Support Service (APSS), in which Liaison Psychiatry is based, provides an integrated Level 3 and 4 service (NICE 2004) to patients across the two sites of the hospital for inpatients, those receiving ambulatory care and outpatients. Other pathways within the service are: directive therapies (e.g. CBT/ Third Wave approaches, EMDR) exploratory therapies (e.g. integrative, existential, psychodynamic) family therapy (primarily working with adult patients with dependent children, and young adult patients) psychosexual therapy.

The Liaison Psychiatry pathway has three main functions: (i) to provide the general liaison psychiatry service to the inpatient wards of the hospital, (ii) to support the APSS Duty Clinician, and (iii) to provide psycho-oncology psychiatry through pre-planned outpatient and day patient appointments.

The specific psychiatric issues and associated risks in a specialist cancer hospital include hyperactive delirium, steroid-induced mania and psychosis, major depression, suicidal planning and intent, desire for hastened death as well as people with pre-existing severe mental illness.

Person Specification

Qualifications

Essential

  • Primary medical degree
  • MRCPsych or equivalent
  • The GMC specialist register for general adult or old age psychiatry
  • If an applicant is UK trained, they must ALSO be a holder of a Certificate of Completion of Training CCT), or be within six months of award of CCT by date of interview
  • If an applicant is non-UK trained, they will be required to show evidence of equivalence to the UK CCT
  • Full and specialist registration (and with a licence to practise) with the General Medical Council (GMC) (or be eligible for registration within six months of interview)
  • MRCP Liaison Psychiatry subspecialty endorsement
  • Higher Degree in relevant subject (MD, PhD or equivalent)
  • Section 12 Approval
  • Registered as a Responsible Clinician as specified by Mental Health Act


Clinical Competence

Essential

  • Assessing & managing risk of harm to self & others
  • Diagnosis and evidence-based treatment of psychiatric disorders in adults
  • Detecting & managing depression and delirium in the physically ill hospital patient
  • Choice of psychotropic medication in the context of physical illnesses and their treatment
  • Assessing and managing the severely mentally ill in the general hospital setting, including the use of the MCA and MHA


Experience

Essential

  • Completed minimum of three years full-time training in an approved higher psychiatric training scheme or equivalent
  • A proven track record in liaison psychiatry at ST or consultant level. Specialised knowledge of liaison psychiatry service provision
  • Clinical expertise in the assessment and management of risk
  • Track record in the training and supervising of clinicians in psychological assessment and nursing development including autonomous practice and prescribing


Desirable

  • Experience in the management of people with cancer and mental health problems, including the assessment and management of complex co-morbidities
  • Experience of oncology / palliative care
  • Post registration experience in general surgery
  • Post registration experience of general medicine
  • Psychotherapy training of any modality
