An excellent opportunity has arisen for a well-motivated Band 6 CPN to work as part of the CMHT for Older Adults.
The post holder will be a member of the Bracknell CMHT for Older People and as such will be mainly based at based at Church Hill House in Bracknell and work within its hours of operation as set out in its operational policy.
We are looking to recruit a CPN who enjoys working with older people to help a multi disciplinary team deliver a needs lead service. You will carry out comprehensive assessments of older adults and offer treatment and support to older people, including those in crisis.
You will have a role in supporting the delivery of the pathway for people with dementia experiencing psychological and behavioural distress and acquiring expertise in this area. As part of the role, you will receive up to date training and supervision from our clinical psychology team as well as mandatory supervision and participation at group supervision.As a member of the MDT, your role will be varied, you will manage a caseload participate in duty shifts, carrying out duty assessments for new service users, attend LAP meetings, and to lead shifts to manage the home treatment team caseload.
The post holder will also offer support to carers and maintain good liaison with GP practices, local care homes and other relevant agencies.

Main duties of the job
- To have clinical responsibility for a case load as specified by the Clinical Team Leader.
- To assess service user needs, plan effectively, monitor and evaluate interventions using evidence-based practice which is relevant to individual needs and within a framework of research and audit.
- Collaborate with other agencies/stakeholders and represent BHFT mental health services at the locality access point (LAP) meetings. To participate in the home treatment team and duty work on a rota basis as directed by the clinical lead.
- To ensure the implementation of Trust and national policies regarding CPA and risk strategies, and in accordance with the current legislation.
- To maintain comprehensive case records in line with Trust requirements.
