The Bladder and Bowel service previously known as the 'continence' service are a specialist team who provide treatment to patients experiencing bladder and bowel problems. We are looking for a dynamic nurse to join our service as Clinical Nurse Lead and help us to showcase our expertise and lead on the life changing care we offer to our patients. Up to full time hours are available and applications for part time hours & flexible working are welcomed.

As well as running specialist nurse led clinics and, we also provide specialist training for our team members in B& B and more widely across the trust.

Based predominantly in Brighton and supporting our teams across the city and the High Weald, Lewes and Havens area of East Sussex, our service operates Monday to Friday and this post has no requirement for working unsociable hours, weekends or bank holidays. You will be able to use your clinical and leadership skills to make a substantial contribution to our teams and patients.

Main duties of the job

Use advanced skills and expert knowledge to assess the physical and psycho-social needs of a defined client group, instigating therapeutic care plans and treatments based on the best available evidence in order to improve health outcomes.

Communicate highly complex information about patient's conditions to multi-disciplinary team colleagues to ensure effective delivery of patient treatments.

Negotiate and agree with the patient, carers and other professionals individual roles and responsibilities with actions to be taken and outcomes to be achieved, referring onto other services or professionals as appropriate. This will include participating in meetings/case conferences in response to the needs of the individual.

Use advanced skills and expert knowledge to clinically lead the team in delivering best practice across Brighton and Hove and HWLH areas of the Bladder and Bowel Service

Use of clinical tools to support clinical assessments and treatments such as Bladder ultrasound scanners.

Provide expert clinical care and health promotion interventions.

Use advanced skills and expert knowledge to identify subtle changes in condition, provide direction to the team and take all appropriate actions necessary to minimise the need for hospital admission.

Ensures effective liaison and communication with other professional groups and outside agencies, acts as an information source and works in partnership in the interest of individuals.

Job description

Job responsibilities

The post holder will be required to lead the provision of a clinically effective, cost efficient service and clinically lead a team of nurses. They will be responsible for the development and delivery of a consistently high quality service, promoting the team as a positive place for staff experience and development.

The post holder will demonstrate at all times a comprehensive knowledgebase and promote excellence in nursing practice in a dynamic healthcareenvironment. As an active part of the team, the post holder is expected toprovide direct patient care when necessary, acting as an expert practitionerand a role model to the whole team. It is essential that they can use initiative and work without direct supervision. They will co-ordinate services to promote optimum health, self-management and effective liaison between agencies.

  • Ability to travel between sites, car owner/driver with access to a vehicle for work purposes

Experience

Essential

  • Extensive clinical experience five years
  • Case management experience
  • Evidence of up-to-date based knowledge and skills, particularly in Long term Condition issues
  • Evidence of ability to maintain and monitor high standards of care
  • Evidence of continuing professional development
  • Eighteen months post graduate experience


Qualifications

Essential

  • RGN first level nurse
  • Post graduate level degree or equivalent
  • Advanced Physical Assessment course Masters Module
  • Non Medical Prescriber or willing to work towards


Desirable

  • Masters qualification
  • Mentoring Course or equivalent
  • Community Nursing Qualification
