We have an exciting opportunity to join our fantastic team at Ipswich IDT, ideal for someone who is interested in innovative and creative patient led recovery and the drive to improve the patient's quality of care, safety, and experience.

We currently have a substantive Band 5 Community Mental health Practitioner post which will be open to Registered Mental Health Nurses, Qualified Social Workers and Occupational Therapists. We are welcoming applications for part-time, full time and job share.

Our team work closely together to support each other as well as giving you autonomy to work with individuals, develop professional therapeutic relationships and support individuals to live as independent and fulfilling lives as possible. We are a recovery focused, multi-disciplinary team, which provide an effective, supportive, and professional service for our service users, carers, and their wider networks.

Currently the team are entering an exciting period of change and transformation, under new leadership. We are looking for people to join us to continue to build a successful service of Ipswich IDT. We aim to continuously improve the service delivery and outcomes as a part of NSFT.

Main duties of the job

The ideal candidate will have an interest in working with a community mental health service. The typical job roles and responsibilities will include:
  • Energising individual who will be excited to be a part of something new and very special.
  • Working as a care coordinator within service users in the community.
  • Creative, innovative, passionate towards providing excellent patient care.
  • Undertaking urgent/routine initial assessments
  • Attending regular MDT meetings and liaising with colleagues from other services.
  • Supporting a caseload via face to face and telephone contacts.
  • Involved service user's family and carers in care planning and decision making
  • Participating in the duty work rota (Within working hours).
  • Participating on the rota for clinical and community depots (if applicable)
  • Reviewing patients care, support within supported accommodation.
  • Opportunities for further learning and training.

Norfolk and Suffolk NHS Foundation Trust is the main provider of specialist mental health services for the counties of Norfolk and Suffolk, serving a population of 1.6 million people.
