As part of the Mental Health Community Transformation we have created two new posts and invite RGN, Paramedic or RMN's to apply who have a interest in Physical Health. A qualification as a NMP would be an advantage.
The Community Transformation will create 2 teams - a Hub Team and Intervention Team, as part of the newly created Hub we will have a dedicated Physical Health Team. The Physical Health Team will consist of qualified and unqualified practitioners, who will provide monitoring of complex medication as well as monitoring for patients commence medication, they will also administer depot injections. We envisage this to be a interesting and challenging role.
Practitioners will work within the wider Hub and Intervention Team in the process of monitoring medication, mental health and risk factors.
There is a ongoing Training Programme across the current 2 teams which we envisage will continue into the new model and all staff are encouraged to attend this training. In addition we support external training and would support NMP training if the practitioner is not already qualified in this area.
Main duties of the job
As part of the Hub your main responsibility will be to lead on the Physical Health Team, but you will be involved in the monitoring patients that sit within the Intervention Team that are taking specific complex medication and ensure that all patients in the intervention team receive 12 monthly minimum monitoring.
The main duties of the successful RGN, Paramedic or RMN's will be to organise and run the Physical Health Team, they will have responsibility for monitoring all blood results, ECG and general Physical Health. Skills and Knowledge of venipuncture, ECG, understanding of blood results and ability manage urgent or out of range results is essential. Practitioners will already be trained or receive training in Clozapine monitoring and the use of the Pochi Machine. Practitioner will have skills in administration of depot injections in the base and home visits. HDAT and Lithium monitoring will also be a main role of the Physical Health Team.
Non-Medical Prescribers will be part of the assessment and commencement of medication and the monitoring of medication within the Hub or Intervention Team. (Prescribing in accordance with Scope of Practice and appropriate supervision will be organised)
The successful practitioners will provide supervision to the colleagues working in the Team, and will receive appropriate supervision from a member of the leadership.
