The Directorate for Nursing -Patient Experience Team is looking to recruit to the post of Self Care Facilitator/EPP Coordinator. This is anexcellent opportunity for a motivated and dedicated individual to join our well established team to support and deliver the Expert PatientProgramme (EPP) for patients with long term health conditions across the Trust.

The post holder will have personal experience of making life adjustments because of a long term condition or caring for someone with long termcondition. Experience of working in an NHS environment would be desirable.

The post holder will be expected to travel across a variety of Trust sites to deliver the EPP programme.

Main duties of the job

  • To facilitate the development of workshops, discussions groups, mind mapping sessions and social events which are aimed at promotingthe aims of the project and development of self-care support provision for people living with long term conditions.
  • To ensure that programmes for patients and carers are accessible to all members of the community and are sensitive to diverse culturalneeds.
  • To work with Patient Experience/ Involvement colleagues to integrate EPP into mainstream Long Term Conditions agenda.
  • To play a key role in ensuring full user (both patient and carer) involvement in their own care by taking on a facilitative or care navigatorrole.
  • To facilitate user groups in relation to service delivery and redesign.
  • To participate in evaluation and service redesign including running focus group and user satisfaction surveys.
  • Organising self-care facilitating courses, including the booking of rooms and refreshments, organising participants transport, orderingmaterials and all contacts with participants.

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436 p.a. pro rata for part time).

Job responsibilities

  • To play a key role in ensuring full user (both patient and carer) involvement in their own care by taking on a facilitative or care navigator role.
  • To contact identified service users on a one-to-one and group basis to promote health promotion initiatives and user empowerment by using the following initiatives:

o Telephone support service

o Specialist Health Professionals

o Email

o Community Visits

o Involvement in the Older Peoples Network i.e. Age Concern

o Collaborative working with community champions i.e. Voices of Experience Healthwatch

o Involvement with Carers Support service

o GPs and Consultants
  • To explore alternative ways for people to access the Expert Patient/Carers Programmes so as to spread the learning of the programme.
  • To establish and maintain close links with other services agencies and voluntary groups.
  • To take the role of a care navigator in directing patients to the relevant pathways of care so as to improve the patient journey.
  • Increase the availability of self-care management programme, and in particular to people from minority ethnic communities.

  • NVQ level 2 or equivalent standard of literacy and numeracy
  • Educated to degree level or equivalent level of proven experience


Experience

  • Experience of working in the NHS or public service related organisation
  • Personal experience of making life adjustments because of long term condition or caring for someone with long term condition
  • Experience of working autonomously


Desirable

  • Experience in Training and Teaching


Knowledge

  • Has a knowledge and understanding of the policies surrounding self care such as the NHS plan, the Public Health white paper and the long- term conditions and self care strategy
  • Ability to develop and implement a project plan
