As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be ensuring children, young people and families are at the Centre of decision making to promote person centred care and outcomes in the implementation of the getting help and getting more help elements of the ITHRIVE framework.

We are looking for clinicians who are creative, highly motivated, shows initiative and is passionate and committed for ensuring high quality services for young people with mental health difficulties.

Main duties of the job

To manage a caseload of young people referred to the CYPS to assist them to overcome the obstacles they face and to achieve their potential. Undertake holistic assessments including risk of the child/adolescent/family needs.

Work in partnership with young people, families/ carers and the Alliance regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.

Act as a lead professional for CYP following initial assessment. Developing appropriate plan of care.

Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.

Have basic IT skills and Data entry skills to enter clinical information on the data bases.

Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.

Through the appraisal and supervision systems identify own training needs and undertake continuous professional development.

I will put the experience of the CYP using the service at the centre of my decision making whilst building a supportive and responsive relationship with colleagues in Surrey emotional well-being service. (Previously Mindsight Surrey CAMHS)

Job Overview:

The Advanced Nurse Practitioner will be a member of the multi-disciplinary specialist team, providing services to children and young people 5-18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems.

The main responsibilities of the post holder will be providing assessments and treatment, either individually or with a member of the multidisciplinary team depending on the complexity of the referral, within both clinic and community settings. The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway the young person is on.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be supporting the team in leading with the implementation and embedding of the THRIVE Framework across the newly created CYPS in West Surrey.

You will have demonstrable expertise working with young people under the age of 18 years.

Degree or diploma level qualification in Nursing, Social Work or Occupational Therapy

Experience required

2-year post qualification experience of working in the community with young people with mental health difficulties and their families.

Experience of and ability to undertake:

ü Mental Health Assessments,

ü Risk Assessment and implementation of risk management plans

ü Use of outcome measures to evidence good practice

This position is suitable for someone who is passionate about providing high quality, evidence-based care and treatment to young people with emerging mental health issues.

Job Purpose

· Work in partnership with young people, families/ carers and the Alliance regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.

Key Responsibilities

• Carry own caseload and undertake holistic assessments including risk of the child/adolescent/family needs, either individually or in identified complex cases jointly with another member of the CAMHS Intensive Intervention Team IIT.

• Provide short term treatment to CYP.

• To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.

• Act as a lead professional for CYP following initial assessment. Developing appropriate plan of care.

• Offer support for Students on placement.

• Co-work with other members of the team MDT.

• Take responsibility for maintaining accurate and timely records of all work undertaken in accordance with Trust policies and Professional body requirements.

• Provide statistical information on all activity undertaken, in line with Trust procedures. Have basic IT skills and Data entry skills to enter clinical information on the data bases.

• Contribute to the teams overall standard of services provided.

• Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.

• Through the appraisal and supervision systems identify own training needs and undertake continuous professional development.

• Ensure high standards of care are provided, and that regard to dignity, individuality, self-esteem and wellbeing of individuals and their families are given. Monitored and evaluated on a continuous basis.
