Occupational Therapist (OT) working within Memory Assessment Service with people with dementia. The OT will work as an integral member of the Memory Assessment Service multidisciplinary team (MDT) and will liaise with other professionals and carers in the assessment, treatment and rehabilitation of service users, enabling maximum independence and quality of life in the community. They will be required to manage a caseload as decided by their manager.

Main duties of the job

To be responsible for the assessment, treatment (individual and/or group) and evaluation of service users referred to Memory Assessment Service who are identified as needing specific OT intervention.

To work as a member of the multidisciplinary team to plan.

To liaise, plan and co-ordinate with other health and social care professionals and agencies to ensure a co-ordinated approach to managing service users care.

To participate in care plan reviews and give advice to service users, carers and professionals on appropriate interventions with service users

To write reports as appropriate and to ensure up to date accurate and contemporaneous records are kept regarding all service user contact.

To provide support and training for students on an informal and planned basis.

To use and integrate as part of practice the principles of Model of Human Occupation Assessments (MOHO) and to produce reports and treatment plans in accordance with assessments.

To provide education and a liaison service for other health and social care professionals and carers regarding dementia and own area of expertise.

To provide assessments and interventions to other teams in older peoples service commensurate with this job description and appropriate to this grade to cover for colleagues absence.

About us

The aim of the Older peoples Community Mental Health team and Memory service in Bury is to promote good mental health and to treat and support those older people with mental health problems. The Older Peoples Occuptational Therapist has a dual role in completing both Occupational Therapy assessments and Memory assessments within the Memory Service.

Main Responsibilities

Clinical

To be responsible for the assessment, treatment (individual and/or group) and evaluation of service users referred to Memory Assessment Service who are identified as needing specific OT intervention.

To work as a member of the multidisciplinary team to plan and provide a range of activities for both individual and groups to meet needs identified for post-diagnostic support for those diagnosed with dementia.

To liaise, plan and co-ordinate with other health and social care professionals and agencies to ensure a co-ordinated approach to managing service users care.

To participate in care plan reviews and give advice to service users, carers and professionals on appropriate interventions with service users

To write reports as appropriate and to ensure up to date accurate and contemporaneous records are kept regarding all service user contact.

To provide support and training for students on an informal and planned basis.

To plan and develop therapeutic groups alongside colleagues and be able to deliver the therapeutic aspects of these groups

To use and integrate as part of practice the principles of Model of Human Occupation Assessments (MOHO) and to produce reports and treatment plans in accordance with assessments.

To provide education and a liaison service for other health and social care professionals and carers regarding dementia and own area of expertise.

Professional

To promote an understanding of the principles and role of OT in Older Peoples Services

To contribute actively to supervision and liaise closely with the Senior OT and line Manager regarding regular planned supervision

To be responsible for identifying, developing and evaluating own professional skills both as an on-going process and formally through Professional Development Plan (PDP) and training as required

to be responsible for keeping up to date with professional issues and research and ensuring that professional standards are adhered to.

To attend the OT Professional Forums

To maintain links and liaise with other OT staff working within the different areas of the Oldham Borough and Trust wide

To attend annual, essential and mandatory training as appropriate

To undertake additional duties from time to time as designated by the Line Manager

To be responsible for developing and maintaining Professional Portfolio and for registration with the Health Professionals Council

Governance

To carry out administrative tasks as allocated by the team manager

To provide data for performance and audit purposes as required by the Trust

To take an active part in any research, audit or other developmental initiatives within the service

To keep records of journeys undertaken for which expenses may be claimed and complete appropriate claim forms as per Trust policy.

To adhere to all Trust Policies

To respond to complaints and incidents as per Trust policy.

To be aware of any potential safeguarding concerns with service users and report the same to senior staff so appropriate processes can be followed.

To be responsible for all resources including OT materials and equipment for the Community OT Service. To ensure that systems for storage, security and stock are in place

Safeguarding

Appointments to regulated and controlled activities require an enhanced DBS disclosure.

All staff have a responsibility to promote the welfare of any child, young person or vulnerable adult they come into contact with and in cases where there are safeguarding concerns, to act upon them and protect the individual from harm.

All staff should refer any safeguarding issues to their manager and escalate accordingly in line with the Trust Child and Adult Safeguarding Policies.

All staff should familiarise themselves with the NICE Guidelines when to suspect child maltreatment 2009.



Professional and Personal Development

The post holder must ensure that they are aware of their responsibilities by attending the Trust Mandatory Training and Induction Programme.

The post holder will be involved in a formal IPDR with their manager at least every 12 months.

The post holder will be expected to take responsibility for their own professional development and will be supported by the Trust to achieve development opportunities as appropriate.

Confidentiality and Information Governance

Confidentiality is of prime importance. In the normal course of duties, the post holder will have access to confidential documents and information relating to patients, service users, staff and contractors, as well as information of a commercially sensitive nature. Such information should not be communicated to anyone outside or inside the NHS unless done in the normal course of carrying out the duties of the post. Disciplinary action will be considered where a breach of confidence has been established.

All information obtained or held during the post-holders period of employment that relates to the business of the Trust and its service users and employees will remain the property of the Trust. Information may be subject to disclosure under legislation at the Trusts discretion and in line with national rules on exemption.

The post holder must maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date.

The post holder must work to the requirements of data protection laws as applicable to the UK, which includes the General Data Protection Regulations (GDPR).

Health and Safety at Work

The post holder is required to take reasonable care of the health and safety of themselves and other persons who may be affected by their acts or omissions at work and to co-operate with the Trust in adhering to statutory and departmental safety regulations.

The post holder is responsible for ensuring that they do not intentionally or recklessly misuse or interfere with anything provided in the interests of health safety or welfare e.g. misuse of equipment.

The post holder is required to contribute to the control of risk and must report immediately, using the Trust Incident reporting system, any incident, accident or near miss involving patients, service users, carers, staff, contractors or members of the public.
