The post holders will work closely with the multi disciplinary team in their locality. They will offer rapid assessment and time limited intensive treatment to people in their own home in order to prevent hospital admission or facilitate discharge from hospital through a recovery approach.

The team will evaluate and consider expanding the service in the future to offer a 7 day service to cover the hours of 8am 8pm. (Practitioners will be required to work 7.5 hours per day including weekends. (shift pattern 8am-4pm & 12noon-8pm).
Our current working hours are 8am 4pm 7 days per week.
