Are you an enthusiastic Physiotherapist with a passion for providing and developing high quality coordinated health, care and rehab services for local people? We may have the exciting and rare opportunity you are looking for in a purpose built rehabilitation unit.
It is based within a brand new 'state of the art' facility which also offers Extra Care Accommodation and a vibrant Community Hub with a full range of services from a community caf, wellbeing services, and much more.
Your role will work alongside health, social care and voluntary sector partners to provide a dynamic integrated model of care for local people and patients.
Using your excellent communication and interpersonal skills, you will develop excellent working relationships with a range of health, care and community partners and expertly attend regular multi-disciplinary team meetings to ensure local people and patients achieve positive outcomes.
We are looking for enthusiastic, compassionate, motivated and creative individuals who can join our vibrant team. An ability to work independently, be a positive role model and demonstrate the Trust's values is essential.
Applicants are required to have a relevant professional qualification and relevant experience of working with adults including undertaking case management, care co-ordination and multi-disciplinary team working.

Main duties of the job
Provide a high standard of physiotherapy input to the designated therapy team. Hours may be required to be worked over 7 days between 8.00am and 8.00pm, depending on service need.
To assess and treat patients within a ward or community setting as an autonomous practitioner
Physiotherapeutic assessment of patients with diverse presentations, physical and psychological conditions, to provide a diagnosis and develop and deliver an individualised treatment programme.
Hold responsibility for own caseload and be responsible for a defined area of the service or a particular patient type
The postholder will ensure their own personal development and participate in the development of others
To supervise, educate and assess the performance of physiotherapy students to ensure the standard of practice and teaching meets the degree standard
To take part in the On Call Respiratory rota if appropriate
