We are seeking a Team Manager for our East South Somerset Community Rehabilitation Service -a forward thinking individual who would welcome the opportunity to enhance their knowledge & skills leading and working alongside teams who are passionate about rehabilitation and supporting people to live their best lives.

The Team Manager will be responsible for the development, management, and delivery of integrated rehabilitation and re-enablement within the communities of South Somerset.

They will work as part of a multi-agency team, and manage complex cases, using evidence-based practice and person-centered principles to assess, plan implement and evaluate interventions.

They will also provide professional governance and leadership, within the sphere of professional practice.

Main duties of the job

To be responsible for the day-to-day operational leadership and management of the IRT team, providing services to patients in accordance with agreed objectives, targets, quality standards, controls and resource constraints.
  • Ensure a high standard of care and treatment for patients and to work with other members of the team and other agencies to best meet the patients' health and social needs.
  • Play an active role in the development of services in line with the Trust's Strategy and Business Plan.
  • Provide clinical support and supervision to staff within the team and to provide clinical leadership.
  • To maintain a clinical caseload of 50% of contractual hours, for which they are personally accountable and professionally responsible
