Older Persons Mental Health Intensive Support Service.

Base: Aerodrome House Gosport

The Intensive Support Team is a new and exciting service working across Portsmouth and the South East region supporting the older persons population of Gosport & Fareham, Havant & Waterlooville and Petersfield and Bordon.

We are seeking positive and motivated staff who are committed to improving the quality of life for others who may be at a point of crisis in their lives who wish to be part of a resilient, welcoming and professional team.

The aim of IST is to implement a model of OPMH care to support older adults with mental health needs both organic and functional who are experiencing a crisis. The service provides time-limited specialist support, assessment, formulation and intervention for individuals who are experiencing a significant decline in their quality of life as a result of complex mental health needs and/or distressed behaviours that may result in placement/home breakdown and/or hospital admission. Those accessing the service present with a complexity of mental and physical health needs which cannot be safely managed by the person themselves, their family, support provider and/or the Community Mental Health Team. IST works with people for a short and focused period of intensive, regular and emergency support for a mental health crisis.

Our work is person-centred and aims to understand the needs and functions of the behavioural disturbance or complex needs the person maybe presenting. We centre our work towards delivering highly individualised programs of care which is built upon positive behaviour support. The aim is to enhance the patient and carers quality of life, thus reducing unnecessary admissions to in-patient hospital or residential care.

Applicants should be highly motivated, have the ability to act on own initiative and have sound assessment and written skills. You will need to be open to change and possess the necessary drive, enthusiasm and commitment to deliver person centred outcomes in a time limited framework. We are looking for a nurses to help us lead this service.

The service is Monday – Sunday 9am – 5pm and based at Aerodrome House in Gosport. Due to the nature of the role it is essential that applicants have a valid driving licence to travel independently across the locality and have access to a suitable vehicle for business purposes
