We’re working collaboratively with patients and families, primary care, social care and the voluntary sector to improve responsiveness and experience of delivering place-based mental health systems that meet local need.
As a result of transformation of our community mental health teams we have this great new opportunity.This role will hold a caseload of complex patients and you will work alongside the CMHT manager and team leader to support clinical decision making in the team and liaison with partner agencies.
The successful candidate will need to be highly motivated, enthusiastic with excellent communication and organisational skills. You will need to be forward thinking, able to engage patients, families, staff and work in partnership with system partners and wider stakeholders. We are a lovely friendly team who support each other.
A high level of clinical expertise is required with a relevant therapeuticand/or nurse prescribing qualification.
