An exciting opportunity has arisen within the Warrington Learning Disability Community Team for a Nurse Practitioner to join our Team.

The post offers a great diverse set of opportunities to workwithin a Team that provides input to adults with a learning disability. The Team also works collaboratively with a number of agencies and providers working with adults with learning disabilities in their own homes and a variety of community based settings.

This post is subject to an enhanced check with adults barred listDisclosure and Barring Service (DBS) Check.

Main duties of the job

We are looking for a compassionate individual who will be able to manage a caseload. The role will include working with adults with complex physical health needs, complex behaviours utilising Positive Behavioural support and/or mental health needs alongside their learning disability. The post also involves enabling and supporting people with learning disabilities to access mainstream services. Experience of working within Learning Disabilities services is essential

The post holder will also demonstrate an attitude which respects and values service users' choice and works alongside them and their carers to ensure they are at the centre of care planning.

Job description Job responsibilities


Responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care.

Act as Named Nurse as required

Ensure that the nursing practices of the clinical area comply with the Trusts policies and procedures

Participate and assist in setting standards of service user care based on sound research findings.

Participate and assist in systematic monitoring and evaluation of nursing practice within clinical area

Carry out nursing care based on the principles of individual care programmes/nursing process in accordance with specialist integrated Care Pathways.

Assist in the development and implementation of integrated care pathways, including appropriate service user assessments and models of care.

Support the implementation and maintenance of team working

Participate in process of potential new admissions to the service, including the assessment of such individuals in accordance with the agreed care pathway and completion of quality reports where appropriate.

Develop therapeutic relationships with service users and carers.

Participate in case supervision to monitor standards of service user care and documentation.

Participate in regular clinical supervision as per Trust policy.

Participate in managerial supervision.

Ensure the ward environment is conducive to promote service users health, safety and well being, thereby preserving and respecting each service users personal dignity, privacy, religious and cultural beliefs.

Incorporate health promotion and health education into your practice.

Ensure record keeping is in accordance with Professional and Trust standards.

Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.

Work in a professional and collaborative manner with the Multi-disciplinary team.

Participate and promote positive intra agency working.

Participate and assist in setting standards of service user care based on sound research findings.
