The post holder will provide a specialist role within the Community Team and will demonstrate higher levels of decision-making and enable the monitoring and implementation of standards of care and to ensure that own clinical practice is driven through clinical governance in line with service requirements and be able to:
Main duties of the job
Support and empower clients, their families and carers to influence and participate in decisions concerning their care by providing information on a range of specialist nursing care and service options.
Participate in local and national learning disability mainstreaming and health equality projects as required, which will include the Learning disability DES, London LD Self-Assessment Framework, Health for all group, 6 lives , Transforming Care and LeDeR action plan.
Promote a positive profile of the Community Learning Disability Service with GP's, Primary Health Care teams and other statutory and voluntary sectors.
Develop with the client, an on-going care plan for health, with an emphasis on preventative measures and promoting well-being (e.g.) facilitating health promotion clinics.
Fully engage in all aspects of the safeguarding adult's process and take lead responsibilities for reporting.
Have the ability to liaise with all members of the Community Learning Disabilities Team and multi-agency as necessary to ensure a cohesive approach to care delivery.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High-Cost Area Supplement
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro rata for part time staff.

The Community Learning Disability Nurse aims to enhance and support people with learning disabilities to live a better and healthier way of life in the community.
To work collaboratively within the Integrated Community Learning Disability Team to ensure the provision of high quality standards of care by effective and efficient management of resources.
To ensure that own clinical practice is driven through clinical governance in line with service requirements

- Experience of providing professional teaching and training of other professionals with clients with a learning disability .
- Experience of initiating , organising and planning interagency meetings and managing clinical services .
- Experience of working with clients who are disturbed or have challenging needs requiring skilled and complex interventions.
- Ability to perform under pressure quickly and effectively , making managerial decisions and following through agreed care pathways.

- RNLD and relevant graduate training . Registration with relevant professional body.
