The post holder will have clinical leadership and management responsibility for the integrated Stay Well @Home Service working across Cwm Taf Morgannwg University Health Board (CTMUHB).

They will work collaboratively with key stakeholders to ensure that a safe and effective service is coordinated and delivered for the population that focuses on avoiding unnecessary hospital and keeping people well within their own homes.

The post holder will be responsible for the operational delivery of the service, and take a lead on the continuing development of the Stay Well @Home Service, leading on operational and strategic planning.

The post holder will be required to work a 12 hour shift pattern, covering weekends on a rota.

Main duties of the job

Lead the clinical and operational delivery of the Stay Well @Home Service.

Lead on and manage highly complex human resource issues.

Responsibility for the clinical governance of the SW@H service.

Develop networks and work in partnership within the University Health Board and Local Authorities and with key partners.

Provide support, advice and training to the team and other departments/services.

Undertake the operational planning role for the Stay Well @Home Service, including business and staffing planning.

As an expert autonomous Occupational Therapist/Physiotherapist maintain personal clinical expertise in practice. Providing clinical support, including complex caseload intervention.

Act as a clinical resource, disseminating expert information to stakeholders.

To audit and bench mark existing good practice.

As a specialist clinician the post holder will undertake audit, research and development of the service.

  • Qualifications and / or knowledge: Degree/Diploma in Occupational Therapy/Physiotherapy, plus Clinical/Management accredited postgraduate specialist course/training relevant to role, or possess skills, knowledge and experience to equivalency. Registered member HCPC. Leadership/Management qualification or evidence of management development e.g. leadership/coaching/mentoring. Detailed knowledge of professional standards and code of conduct. Evidence of knowledge/experience or qualification in research and audit. Knowledge of the requirements of clinical governance. Knowledge of Health Issues affecting the local population and strategies in place to support improvement in these areas. Knowledge of health and safety. Evidence of professional development.
