We are looking to appoint an Occupational Therapist Band 6 to join the Intermediate Care team which is part of the Neighbourhood Rehabilitation Service (NRS) working across Barnsley Community.

Candidates who do not currently demonstrate this level of knowledge & skills but are willing to work towards the achievement of the below competencies are invited to apply as a Band 5 Aspiring Occupational Therapist in the NRS.

To work as a Band 6 Occupational Therapist in NRS, in addition to BSc (or equivalent) in Occupational Therapy & HCPC registration, you will be able to demonstrate:

Ability to manage a varied caseload of people with rehabilitation needs in the community

Provide specialist, high quality assessment & rehabilitation to improve health & independence, to enable earlier discharge from hospital or prevent hospital admission

Assess, manage & provide treatment to individuals with complex & /or multiple needs, working closely with the MDT providing supervision/support to other staff

Ability to deliver education regarding community rehabilitation

Main duties of the job

Be responsible for the assessment & provision of a specialised programme of rehabilitation, meeting the specific needs & agreed goals of your patients to facilitate their rehabilitation & optimise recovery potential

Manage a clinical caseload with autonomy

Monitor & evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions & service delivery

Develop programmes of care to support discharge

Contribute to clinical leadership/supervision facilitating a culture of learning/reflective practice

Have high level communication & excellent interpersonal skills to negotiate & establish collaborative partnerships with patients & care professionals

Ensure compliance with clinical governance being responsible for ensuring Infection Control policy/procedures are followed

Assist in service development & projectsas we strive to transform & modernise patient care

A vehicle to undertake travel is essential & the ability to work flexibly including evenings, weekends & bank holidays

JOB SUMMARY

To provide a specialist Occupational Therapy Service for the treatment of patients within the Integrated Neighbourhood Team.

To provide supervision, training and development of Basic Grade Occupational Therapists, other health care professionals, support staff, and of OT students, if present.

To undertake high quality evidence based assessments and treatments of patients within the relevant clinical area. These patients may have complex needs and multiple pathologies.

To participate in planning, development and evaluation of Occupational Therapy services within the clinical areas.

To assess occupational performance, plan and carry out goal orientated intervention programmes.

To provide Occupational Therapy advice to other professions.

To assist in administration, supervision and development.

KEY RESULT AREAS:

To prioritise and manage a significant caseload of patients within the Integrated Neighbourhood team.

To assess for physical, cognitive and perceptual deficits in patients and to carry out assessment, treatment and using critical analysis to treat patients through goal setting, which is evaluated and adjusted throughout the patients period of care.

To assess the individual in the most appropriate environment and to be responsible for the assessment, prescription and evaluation of specialised equipment, taking into account safe use/risk to patients and carers.

To receive and communicate complex, sensitive and contentious information in a clear and effective manner with patients/family and carers. To overcome barriers such as hearing loss, fear, behavioural issues, language problems in order to maximise rehabilitation potential.

To collect and effectively communicate information to relevant agencies, acting for and on behalf of, the patient and carer, in order to obtain equipment or services, in order to maximise collaborative working.

Deputise for Band 7 in the management of the services for duties identified by them.

To attend meetings, provide detailed written reports, risk assessments, discharge summaries and other relevant information and recommendations. To communicate effectively within the Multi-disciplinary team, contributing to the decision making regarding discharge, planning and long term management.

To participate in the induction and training of staff within multi professional, multi agency teams.

To provide formal and informal supervision of junior grades of staff and rehabilitation assistants.

To undertake the education of Occupational Therapy students whilst on clinical placement within the department.

To comply with professional and trust policies and procedures, to take individual responsibility towards health and safety issues, undertaking risk assessments as appropriate.

To be responsible for maintaining own continuing professional development in line with trust policy and professional guidelines, and to undertake appropriate training and development to meet the needs of KSF/SDR.

To participate in service developments in areas related to evidence based practice, service delivery and clinical governance.

To contribute to the Occupational Therapy team, joint therapy team, multi-professional team and older peoples service development.

To be responsible for maintaining accurate and comprehensive patient treatment records in line with COT/BAOT guidelines.

To provide supervision, training and development of Basic Grade Occupational Therapists, other health care professionals, support staff, and of OT students, if present.
