Low Secure Services have an exciting opportunity for a highly motivated and experienced registered mental health nurse. The service provides a therapeutic inpatient environment to service users with mental health problems within a low secure setting. Service users may have a forensic background and are detained under the Mental Health Act. Low secure services focus on delivering the highest quality nursing care to complex individuals using a variety of therapeutic interventions to assist in reducing risk and enabling the service users to manage their own risk behaviours, as well as recognising signs of relapse and what support will be required to ensure the safe delivery of care. Our aim is to plan for the most appropriate discharge pathway tailored to each individual's needs and being a band 5 RN within our team you will work collaboratively within the MDT to support our service users move through there recovery pathway. Collectively we strive to achieve the best outcomes for service users and enjoy working collaboratively. You will work closely with the charge nurses and clinical team manager for your ward and you will be expected to be an ambassador not only the ward you are assigned to, but for the Forensic service as a whole.

Main duties of the job

You will be well organised, have excellent communication skills and able to form good working relationships with the MDT and service users. You will need to coordinate a shift and allocate necessary duties to the team you are working with. You will take the responsibility of named nurse and support the service users by working collaboratively not only with the MDT but with the service user themselves in care planning, risk assessments and developing appropriate recovery outcomes. You will have a shared responsibility to ensure that the team has effective clinical supervision and will be expected to ensure your compliance with training and revalidation is up to date and effective. You will support the service in delivering care using evidence based practise, up to date interventions and support the delivery of care to a high standard. You will assist in the risk assessment/management and care planning for service users collaboratively with both internal and external stakeholders such as families and carers, probation, MoJ, social workers, MAPPA lead for the Trust, to name just a few. You will ensure that the service users needs are assessed with consideration given to all aspects of the individual - not just their mental health and risk profile, but their physical, emotional, legal, spiritual, social and psychological needs as well.

Job description Job responsibilities

As a band 5 you will perform the duty of Named nurse/primary nurse to several service users on the ward. Providing support and high level of care to all service users. This will include Formulation of collaborative care plans and risk assessments, supporting the service user through the whole CPA process whilst producing reports and care plans following the CPA. You will ensure all documentation is accurate, professional and defensible, whilst ensuring your case load care plans, S132 rights and risk assessments are up to date and reviewed on a regular basis. You will prepare ward round updates and attend ward round to review all service users on the ward. Therapeutic 1:1 time and escorting service users into the community will also be an expectation.

You will be able to give and receive handover and perform as a Shift co-ordination, whilst allocating duties and supporting junior staff. You will be able to make quick decisions with sound clinical judgement.

You will attend local meetings on the ward such as Clinical governance and team meeting and participate accordingly. You will receive both management and clinical supervision and provide clinical supervision to junior staff.

You will assume the role of security nurse when allocated to maintain a high level of both physical and relational security with in the ward. Escalating issues when they arise
