This is an exciting opportunity for a Care coordinator to join our Community Mental Health Team
  • To manage a caseload of people with complex mental health and social care needs, using evidence-based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
  • To have care-coordination responsibilities for an agreed number of clients on enhanced Care Programme Approach (CPA) within the team.
  • To participate in the assessment of people referred to the team and present findings to the multi disciplinary team.
  • To provide leadership for junior staff, multi-disciplinary support staff and students through formal supervision, education and appraisal.

Person Specification Qualifications Essential
  • Registered Mental health nurse or registered social worker
Desirable
  • 1.Post registration qualification in a clinically orientated area 2.Psychosocial interventions qualification 3.Qualification in leadership or management
Experience Essential
  • 1.Post registration experience. Recent experience working in either adult mental health or CAMHS.
  • Experience of and an understanding of working in a multidisciplinary, multi-agency or complex care organisation
  • Experience of working with service users with risk related behaviours
Experience Desirable
  • Experience of working in a range of clinical settings
Experience Desirable
  • 3.Experience of working with people with early onset psychosis and their Families
  • 4.Working in a community setting and lone working
  • 5.Lived experience of Mental health issues or a caring role of someone with mental health issues.
Knowledge and skills Essential
  • 1.Specialist knowledge of impact of psychosis on young people
  • 7.Ability to work autonomously and set own priorities in a demanding environment.
  • 11.High level interpersonal skills for effective working relationships and clinical effectiveness
Desirable
  • 3.Knowledge of evidence and value-based interventions for people with first episode psychosis
  • 4.Knowledge of evidence and value-based interventions for people with co-existing drug and alcohol problems and psychosis
