Devon Partnership Trust are looking for a Band (6) Senior Social Worker to join the Recovery and Rehabilitation Services in our Adult Directorate.

These roles are new to DPT and are pivotal to our aspirations to develop high quality Rehab services across the Trust, in line with implementing the Community Mental Health Framework.

The role will involve working as part of a wider multi-disciplinary team (MDT) supporting people through the Rehab pathway in both inpatient community settings upon discharge, using your expert Social Work skills and knowledge in assessment, support planning and commissioning in line with the expectations of the Care Act 2014 and S117 aftercare planning.

About you
We are looking for enthusiastic Social Work professionals across Devon who can evidence a commitment to strengths-based practice, a real pride in their profession and passion for delivering Social Work and Social Care interventions as part of a varied and skilled MDT.

You will be an experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England. You will have completed the Assessed First Year in (post-qualifying) Social Work practice, if appropriate, and can demonstrate a commitment to ongoing professional development as we are committed to supporting you to look at post-qualifying opportunities during your career with us.

Bringing a comprehensive working knowledge of the issues currently impacting on social care delivery, you will be up-to-date with policy and practice development and be confident in your knowledge base and application of the Care Act 2014.
