Band 6 Living Well Coordinator- based at Sobell House/Katharine House Hospice 22.5 hours a week .

We are seeking an enthusiastic, energetic, creative and proactive Living Well Coordinator to join the Living Well team at Sobell House and Katharine House Hospice. This is an exciting opportunity to work alongside the other members of our multidisciplinary team, to coordinate, develop and provide a wide range of wellbeing focused support to our caseload. The post holder will arrange, co-ordinate and develop the Living Well programme of sessions, activities and courses as well as the current provision of online/remote support.

We would welcome applications from registered Nurses with appropriate, relevant qualifications and experience.

Main duties of the job

The Living Well service aims to enable and empower individuals to live well by equipping them with skills and knowledge to help them achieve their goals and to live day to day with a sense of purpose and wellbeing.

We're looking for an individual with excellent interpersonal and communication skills - a team player committed to collaborative working with colleagues and patients to promote self-care. The successful candidate will demonstrate innovative and confident teaching skills and an ability to assess patients' health and psychosocial needs to plan appropriate care through shared decision making. In return we will be able to offer a supportive and friendly working environment, with access to teaching, clinical supervision, involvement in research and opportunities for relevant postgraduate study.

Please note, this is a split post between Sobell House Hospiceand Katharine House hospice so some travelling will be required .

The service is open Monday to Friday, staffed flexibly to cover the service from 08:00-18:00 with occasional workshops run at the weekend and in the evening. We strongly encourage you to arrange an informal visit. Please give us a call to find out more:

Job responsibilities

Operational Management

Support with the operational management of the Living Well services, including:
  • Coordinating practical and emotional wellbeing support to patients, their families, friends, and carers.
  • Facilitating sessions and courses, both within the hospice and at sites across the community as well as online/remote.
  • Designing and scheduling sessions and courses and further developing the Living Well services in collaboration with Living Well Lead and wider MDT.

Clinical
  • Accept referrals to agreed criteria.
  • Work in partnership with other professionals to deliver co-ordinated programmes of care effectively and referring patients to other practitioners as required.
  • Assessing health and psychosocial needs of individual patients, and provide care to meet these needs, through a process of shared decision making.
  • Undertaking 1:1 and MDT assessment of patients, establishing personalised goals and assessing suitability for available activities.
  • Providing physical support to patients accessing services if required. This may include moving and handling.
  • Communicate with patients in ways that empower them to make informed choices about their health and health care.

Person Specification

Qualifications

Essential

  • Nursing/Occupational Therapy qualification/degree or equivalent relevant professional qualification
  • Registration with professional body - e.g. NMC


Desirable

  • Specialist skill that would enable co-ordinator to lead sessions both face to face and remotely
  • Palliative care module at diploma/degree or Masters level


Knowledge

Essential

  • Demonstrates up to date knowledge in palliative care


Desirable

  • Evidence of being able to work across organisational boundaries


Experience

Essential

  • Experience of working as a Band 6 nurse
  • Experience of undertaking complex holistic assessment


Desirable

  • Co-ordinated or led volunteers in the past


Personal skills

Essential

  • Ability to problem solve in a creative and flexible way, produce new ideas and insights and use own initiative.
  • Experience of working as part of MDT
