We are looking for a Band 5 Community Nurse to work within our St Neots Team.

In these roles, you would be working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators. Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.

Main duties of the job

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

We are looking for people who:
  • Have experience of working in the Community
  • Are educated to RGN level 2
  • Are able to demonstrate a commitment to high quality care and service provision
  • Have the ability to work flexibly and as part of a team
  • Have the ability to assess and formulate care plans
  • Are able to teach and support student Nurses and Health Care Assistants
  • Have enhanced communication and inter personal skills

You will be responsible for:
  • Keeping the patient at the centre of care, ensuring patient and carer participation in decision making
  • Providing a high standard of care
  • Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
  • Working as part of a 24/7 service and multidisciplinary team

  • To work as an autonomous practitioner with the integrated multi professional team
  • To be accountable for assessing interpreting, planning, implementing and evaluating treatment to patients within professional guidelines
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making
  • Provide support and education of peers, new staff, non registered staff, and students
  • To provide high standards of nursing intervention within a patients own home, this includes lone working , with access to specialist and advanced practitioners when required


Person Specification

Education / Qualifications

Essential

  • RGN level 1, RGN level 2
  • Evidence of professional development


Desirable

  • Mentorship qualification or equivalent
  • Management of leg ulcer qualification


Experience

Essential

  • Community experience


Desirable

  • Primary care experience and experience in wound care


Skills & Abilities

Essential

  • Ability to work flexibly as part of a team
  • Good documentation skills
  • Able to assess plan and implement care and formulate care plans
  • Support Integrated Care Workers and teach and support student nurses and HCA's.
  • Ability to safeguard and promote welfare of children and young people/ vulnerable adults.
  • Good communication skills
  • Appreciates the significance of safeguarding and interprets this accurately for all children and young people/ vulnerable adults whatever their life circumstances.

  • Understanding of clinical supervision

  • Kneel, bend & stoop, and work in cramped environments
  • Travel around work base locality
  • Move and manoeuvre people and equipment
  • Manoeuvre patients using safe techniques in accordance with care plan.
