An exciting opportunity has arisen within the Chandlers Ford Therapy Team to appoint an enthusiastic and motivated Occupational Therapist.

We are seeking a Band 6 Occupational Therapist to work within our Integrated Community Team, visiting people in their own homes or clinic setting. We are currently working to develop and expand the UCR (Urgent community response) model of care whilst continuing to deliver rehabilitation to adults living in the area who have long-term or complex health needs.

You will work closely alongside the nursing and frailty teams and wider Primary Care Colleagues to prevent hospital admissions, support patients being discharged from hospital, palliative care and promote self-management for patients with long term conditions.

We are looking for a skilled and motivated practitioner who is dynamic and keen to work as part of a team to achieve the best for their patients. The successful candidate will need to have good communication skills, the ability to work well within a team and be able to demonstrate good time management and prioritization skills. The post holder will participate in the development, training, and support of the multi professional team and students. Applicants must be a registered Occupational Therapist and have full access to a vehicle for work and possession of a full driving license is essential.

Hours- 37.5 hours per week (Part time hours will be considered)
