Join our brand new, developing service as an Advanced Clinical Practitioner! Nursing AND Physiotherapy ACP role available!

The respiratory element of the Somerset Hospital@Home programme isdesigned to facilitate early/supported discharge of patients from acute medical beds and to prevent hospital admissions of patients with unstable respiratory disease in the community, so creating a 'virtual ward'. There is also to be a programme for patients with hard-to-manage breathlessness. A mix of Band 4/6/7/8 staff will provide the respiratory service, supported by senior medical professionals, and work across the four Somerset neighbourhoods. The respiratory team will align closely with colleagues in other evolving community services (notably the frailty H@H virtual ward team). This is a 7 day a week service.

Please note, this is a community post covering one of four neighbourhoods within Somerset (Yeovil, Taunton, Bridgwater or Mendip) therefore you must have access to your own vehicle with a UK driving license (We will support and pay towards mileage costs)!

Main duties of the job

You will be responsible for the rapid assessment, diagnosis and treatment of individuals within the respiratory service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses. Typically, you will be required to manage patients with COPD, asthma and other airways' disease (including bronchiectasis), lung infections (including COVID surveillance) and other miscellaneous lung conditions. Patients will be assessed at home, in hospital, via telephone/video and there will be facility to use remote monitoring equipment.

Job description Job responsibilities

You will plan and manage complete episodes of care in collaboration with senior medical colleagues and others when required. You will be required to delegate and refer as appropriate to ensure timely, effective management, and to optimise health outcomes in line with the evidence base and nationally recognised best practice.

You will be expected to set-up, with your colleagues, and supervise, a daily morning Board Round/huddle to plan the days activity, to consider at-risk cases, to troubleshoot and ensure there is an ongoing and positive team dynamic. You, alongside the rest of the team, will attend a weekly MDT meeting, to be chaired by a consultant respiratory physician, for the purposes of complex case review, training and education and discussion of service matters. You will ensure that there is a weekly physio-led breathlessness service running out of Musgrove Park and Yeovil hospitals.

You will play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.

You, alongside the senior medical professionals, will promote and implement service and policy development and redesign, impacting beyond your own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care. Notably, you will be required to develop close working relationships with colleagues in the Frailty service as there are commonalities of care that can usefully be shared.

You will be responsible for ensuring high quality learning environments for all disciplines within a context of multi-professional education and training that is responsive to future changes and challenges, reflecting and embedding current trends and feedback (e.g. Health Education England, HEE, National Education & Training Survey, NETS), leading to, and supporting, the maintenance of a learning environment to produce learners that are Fit for Our Future & workforce plans, including meeting The Long Term Plan patient pathways

Person Specification Education / Qualifications Essential
  • Professional Registration with either NMC, GPhC or HCPC.
  • Evidence of MSc level education. Such as:
  • Evidence of Continuing Professional Development
  • Relevant role specific qualifications (i.e. Advanced Resuscitation -ALS/EPLS/ATLS or surgical skills qualification)
  • Non-Medical Prescribing if appropriate to registered profession / role.
  • Leadership/management course or validated portfolio evidence (if not part of MSc level qualification)
Desirable
  • Ability to communicate effectively with all stakeholders including to service users, clinicians, and senior leadership teams. This will require proven ability and leadership skills demonstrating motivational, visionary, diplomacy and negotiation skills.
