An exciting opportunity has arisen for an Operational Lead for Pershore & Upton Neighbourhood Team. The team support patients registered at Pershore Medical Practice, Abbotswood Surgery and Upton Surgery
The role entails the operational leadership for the team to reduce avoidable Hospital admissions, facilitate Hospital discharges and provide timely anticipatory and planned care as well as exceptional end of life care. Having oversight of the Intensive support caseload is key in order to maintain capacity within the service, ensure that delays are managed and that a reablement approach is embedded to maximise independence. Managing the urgent community response work is also a priority, working closely with the team to ensure that 2-hour targets are met.
You will work closely with the Clinical Lead, Physiotherapy Lead, OT Lead and Admin Lead and be supported by the service lead. The post will also involve deputising at senior management meetings and working closely with the other Neighbourhood Operational Leads.
The role involves line management responsibilities for direct reports consisting of Senior Community Nurses and a senior Administrator. Managing the staff roster to ensure that all shifts are covered is a key part of the role as well as organising bank and agency staffing where necessary. You will work closely with the Integrated Service Lead to ensure that recruitment is managed within budget and HR targets and service targets are achieved.
Main duties of the job
The post holder will be expected to demonstrate strong interpersonal skills, and be able to prioritise and manage own workload using their own initiative to include the following:.
Clinically lead, clinically coordinate and develop the provision of an effective and efficient integrated care service for patients primarily in the older adult population who have multiple co-morbidities within the extended primary care pathway of a designated locality.
Provide day to day clinical leadership, clinical support and clinical supervision of the Neighbourhood Team Occupational Therapy (OT) staff within the integrated team as well as supporting the Integrated Neighbourhood Team Leader and Integrated Community Service Lead to provide a seamless service right across the patient pathway.
Support the effective triaging of patients referred to the Integrated Neighbourhood Team on a daily basis, supporting the coordination of care delivery across the patient pathway.
Provide expert clinical holistic assessment and evidence based pro-active and reactive health care interventions to support patients and their carers to achieve maximum health, wellbeing and independence.
