An exciting opportunity for an Advanced Clinical Practitioner with Frailty experience to provide comprehensive geriatric assessment (CGA) for frail older adults at Bassetlaw General Hospital and surrounding community. This is a great opportunity to be part of a team developing the Frailty provision which includes integrated working between the Emergency Department, wards and primary care & community services.
Main duties of the job
Reducing the number of hospital admissions by providing clinical expertise and working collaboratively with MDT colleagues in primary and secondary care.
Utilising the most appropriate patient pathways including virtual ward and other community services for patients who can safely be discharged into the community, contributing to a reduced length of stay, improved patient experience and outcomes.
CGA and high quality care for frail older patients within the Emergency Department, ATC and Ward A4. This will encompass the skills of advanced clinical assessment, physical examination, diagnosis and treatment within an agreed scope of practice.
Due to the Trusts commitment to continuous improvement, it is likely that this post will evolve over time. The role will be subject to regular appraisal and any amendments may be reviewed in the light of experience and developments within the service.

- 5 years post registration experience
- Highly specialist knowledge underpinned by theory and experience
- Complex reasoning, critical thinking, reflection and analysis to inform assessments, clinical judgements and decisions.
