To provide a unified and integrated management structure for all staff in the Community Mental Health Hub to ensure that the team work to the agreed operational policy offering a high standard of integrated assessments, treatment and a social care service to people presenting with mental health problems.

To develop a model of working with service users with complex, long term needs, ensuring that services to those practices are provided to the highest possible standard and that there is a constant team focus on preparing service users, using the Zoning model, for eventual discharge.

To work closely with the Primary Care Network and with local GP practices, to ensure patient flow is maintained throughout the care pathway.

To work closely with inpatient services and Crisis Resolution Team.

Main duties of the job

  • To be the operational manager for the Community Mental Health Hub, responsible for the workload of the team and ensuring that priorities are met.
  • To have operational responsibility for the relevant sub-teams associated with the community Hubs
  • To take a lead on the health services provision within Community Mental Health Hubs
  • To line manage CPNs, AMHPs/Social Workers, OTs, unqualified staff and to line manage psychologists where required.
  • To ensure robust delivery of an intervention based model of care including group programmes and discipline specific treatment options for service users
  • To ensure systems for the delivery of CPA and Personalisation are in place and that they are made available to service users in the team
  • To work with in the relevant legislative frameworks |(e.g. Mental Health Act (1983) Mental Capacity Act (2005), Deprivation of Liberty Safeguards, Care Act 2014 etc)
  • To promote a culture of service user choice and control within team members daily practice to be monitored and developed through supervision and appraisal
  • To ensure that the team works within the relevant frameworks and policies for safeguarding vulnerable adults

  • To ensure that systems are in place for monitoring allocation, caseloads and clinical review processes
  • To ensure that clinical supervision is given to all professional staff in the team and to ensure that staff work to high standards of professional practice.
  • To demonstrate a high level of clinical competence both in relation to supervision of staff and direct face to face contact with service users
  • To be a member of the CMHH management team. To advise the Community services manager regarding the delivery of community mental health services and be involved in identifying the resources needed in the team.
  • To ensure that the service works in co-ordination with other community and Acute In-Patient services inclusive of Bed Management
  • To ensure effective liaison with other relevant agencies
  • To maintain and develop administrative procedures regarding the recording of case records.
  • To ensure that service users are involved in the development of the team and local services
  • To facilitate the development of effective supporting structures for carers in line with CNWL and LBH guidelines



Person Specification Essential Essential
  • oProfessional qualification in mental health (Mental Health Nursing (RMN); , Social Work (must have HCPC registration)
  • oDegree/Masters in specialist field or appropriate experience.
Desirable
  • oDegree/Masters in specialist field or appropriate experience.
Experience Essential
  • oExperience of managing a multi-disciplinary staff team, including line management, supervision responsibility and operational management of services at band 7 in a mental health setting
  • oExperience of multi-professional collaboration at senior level, including partnership working with other agencies.
  • oExperience writing reports and delivering presentations to a high standard and to senior staff
