Main duties of the job

As a Registered Nurse with current NMC registration you will have significant post qualification experience in the field of heart failure. With strong communication and organisational skills and a flexible approach, you will work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes.

You will demonstrate the ability to effectively and efficiently assess, plan, provide and evaluate evidence based nursing care for patients.

You will demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.

You will have a commitment to professional development and contribute to a culture of continuous improvement.

You will organise and prioritise your own day- to-day work tasks and activities and delegate tasks to junior colleagues.

1. To adhere to and apply the Nursing & Midwifery Council (NMC) code of conduct, and all other relevant guidance from the NMC and follow the Trust values.

2. To be professionally accountable and responsible for all aspects of own work.

3. To establish and maintain effective working relationships both within the Trust and with external organisations in order to meet service requirements and support and demonstrate the Trusts equality values.

4. To assess patients and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. The specialist care/support plans may include the following:

Promoting independence/self-management and maintaining safety

Increased functional ability

Improvement in health and wellbeing including mental health

Interventions in disease/condition management

Prevention and reducing of health inequalities

Acute hospital admission avoidance

Early supported discharge

Case management and treatment /care plans

Specialised interventions around specific long term conditions and clinical need

Palliative and End of Life

Improvement in health and wellbeing including mental health

Ability to work without supervision or direction

5. To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider patients circumstances and preferences in order to implement and adjust programmes of care and intervention

Person Specification

Qualifications & Training

Essential

  • Degree in nursing or equivalent
  • Registration with NMC
  • Post Graduate studies in relevant nursing field such as end of life care, long term conditions/palliative care or related/relevant subject
  • Mentorship qualification (998-997)
  • Venepuncture
  • Post qualification courses in nursing
  • Attendance at relevant networks e.g. Special Interest Groups
  • Up to date field work/pre-registration student training. i.e. Cardiac/ Heart Failure specialism areas.


Desirable

  • Modules at Masters level
  • Non-medical Prescribing V300 or working towards
  • Evidence of further clinical study


Experience

Essential

  • Relevant experience to the role including assessment and management of long term conditions
  • Ability to manage time and caseload effectively
  • Supporting service users, families and carers in dealing with Heart Failure and its associated complications. Working in a multidisciplinary team


Desirable

  • Experience of negotiation and conflict management in clinical situations
  • Experience in leading and supporting junior staff
  • Experience of involvement in service improvement
