The Asthma Nurse will be working with the Royal Stoke Adult Asthma Team, liaising with GP's, practice nurses/nurse practitioners to support the screening, planning and monitoring of patients diagnosed with severe asthma. The successful post holder will be expected to take on a caseload of patients, deliver clinics both virtually and face to face, and to support and coach patients to become independent. The successful post holder will join an expanding team to deliver high quality care to patients with asthma both on an inpatient and outpatient basis assisting to identify those patients who may be eligible for biological asthma treatments.
Main duties of the job
Taking on a caseload of patients to support from referral onwards: contributing to planning diagnostics work-ups to support asthma diagnosis and confirm eligibility for biologics.
Completing and sending necessary information to the severe asthma centre MDT to support patient initiation on biologics.
Involvement in biologic clinics administering biologic injections to patients, assessing the response, delivering questionnaires and requesting relevant investigations.
Involvement in adherence clinics helping patients to use medication appropriately, checking inhaler technique and improving patients understanding of medication.
Coaching patients to become independent.
Remote and face-to-face support for patients.
review inpatients following an acute exacerbation of asthma in line with guidelines.
The post holder will be able to assess, monitor and provide management plans to patients in nurse led clinics (virtual and face to face).
Assist with training patients to self -administer medication.
completion of clinical proformas for local and regional multi-disciplinary team meetings and attend MDTs.
Assist with entering patient data onto the national BTS Severe Asthma Registry.
