Welcome! The Telford IAPT service is a busy caring team consisting of Administration staff, PWPs, CBT Practitioners, and Counsellors. We serve the people of Telford which is an exciting, diverse and growing modern town, surrounded by the stunning Shropshire countryside.

We are currently recruiting for:

Role: 2 x Community Mental Health Practitioner (IAPT)
Grading/salary: Band 6
Permanent
Hours: 37.5 per week
Base: Fuller House, Hall Court

This is an exciting opportunity to be part of developing a new role within Telford & Wrekin IAPT to support therapists and help people whilst they are waiting for therapy.

For this role, we are looking for either a registered Mental Health NurseORa statutory regulated mental health professional (e.g. social work, OT)

The job role will include:
  • Working within the IAPT stepped care pathway to undertake the identification of care needs through assessment, as well as signposting and short term care planning for people referred to the IAPT service, who may not meet the criteria for psychological therapy.
  • Delivering a high quality clinical service to patients and their families to people with unidentified complex health and social care needs.
  • Provide short term / focussed clinical intervention, where appropriate and appropriately care planned.
  • Coordination and management of an identified caseload with access to regular and robust supervision.

    IT literacy and computer confidence is essential to all members of our team, all of our communication from service user records to how we interact is all computerised. We use IAPTus for all of our servicer user records therefore experience of this or other patient management systems would be useful.
