Main duties of the job
We are looking for a Band 6 Team Leader/Caseload Manager to join our Community Nursing Team based in Deal.
You will work as part of a close-knit team who are dedicated to the health and wellbeing of people inDeal. Dealis made up of a diverse patient group with complex needs.
As aBand 6 Team Leader you will undertake holistic assessments, planning appropriate care and delivering clinical interventions to meet the various needs of patients on your caseload.
You will offer health education advice and promote independence in all consultations so you can support your patients to manage theircomplex long-term conditions appropriately and maintain their independence.
Additional Working Pattern: Monday - Friday 8-4 with 1 x 10-6 1 x weekend per rolling roster month with days off in the week either side of the weekend

- Registered health care professional (NMC registration) Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Specialist qualifications relevant to area of practice (examples specialist practitioner district nursing, non-medical prescribing mentorship, coaching, clinical expertise)

- PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study Working towards Master level study
