We have a temporary Band 6 Learning disabilities nurse post available until 31st March 2023 whilst one of our Band 6's is working elsewhere. We are looking for a part or full time Band 6, who is enthusiastic and motivated, and able to make a difference in a short space of time. We work with adults (& young people with learning disabilities in transition) to access the appropriate health input they need - this might be to assist with their physical health and access to primary or secondary care, to support with their emotional wellbeing or to help people with their mental health.

There is huge variety in the workload - the team work with young people in transition, those who need more intensive support and general health facilitation - you will be working with people in a number of different settings including their own homes, family homes, residential and supported accommodation provision or hospitals.

Main duties of the job

The post holder will work collaboratively with people with learning disabilities, their carers and professionals and agencies to provide a high quality, comprehensive and dynamic specialist nursing services with the central aim of maintaining and improving the health and wellbeing of young people & adults with learning disabilities. This will include but is not limited to, working directly with service users who have complex healthcare needs, supporting others to act as Health Facilitators, supporting health providers to make reasonable adjustments to care pathways and engaging in a range of health promotion activities.

Job description Job responsibilities

- Within a multi professional & multi-agency context applynursing skills, knowledge and leadership in the provision of aperson-centred health service that promotes and facilitatesthe health and well-being of people with learning disabilitiesand associated complex needs.- Provide Health Screening and Health facilitation.- Provide health interventions to individuals as identified.- Undertake individual therapeutic sessions to achieve planned health outcomes for service users- Lead the assessment, development, implementation andevaluation of the Health Action Plans for people with learningdisabilities who have associated complex healthcare needs. Promote and assist people with learning disabilities, carers,health and social care professionals in the implementation ofhealth action plans.- Maintain continuing responsibility for a designated group ofservice users with learning disabilities who have complexhealth care needs.- Provide Clinical Case Management to service users withlong term complex healthcare needs.

The Governments' proposal to mandate Covid 19 vaccines for all health care workers is currently under review.
