We are delighted to offer an opportunity to join our team as a Level 2 Social Worker within our services based around the Staffordshire Moorlands.
We are looking for a highly skilled practitioner with in-depth knowledge of safeguarding and complex case managementand has an excellent working knowledge of the Care Act 2014 and Mental Capacity Act 2005.
You will be working in one of the largest integrated health and social care providers putting people at the heart of what we do, empowering people to improve care & wellbeing and delivering better health, better care in partnership. There is a strong emphasis on strength based practice, forging links within local communities to provide an outcome focused, person centred service as we move towards a meaningful integrated care system.

Main duties of the job
You will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal literacy, Strength and asset based practice, personalisation and social inclusion is embedded and applied effectively throughout their work.
A comprehensive knowledge of the services available to support and promote independence in the home environment is essential. The work is varied and challenging due to the geographical nature of the district and there is strong ethos of adaptability and innovation in the teams.
You will have a flexible approach, excellent communication and organisational skills and have the ability to develop excellent working relationships with multi-disciplinary teams and partner agencies.The role will involve assessing accessing and implementing a range of social care services, and providing guidance/advice and information for adults with acute or increasingly complex long term physical health, mental health needs and for older people and carers.

Due to the impact of Covid we have changed the way we work with more flexible and remote working and we are working to a hybrid model at present, with part-remote and flexible options available in line with our service needs. This gives our Social Workers a quiet space and plenty of time for reflection. It has also increased our Social Workers autonomy and has a positive impact on wellbeing and work-life balance.
Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
- Ensuring current registration with the Professional Social Work regulatory body at all times.
- To hold/to undertake and achieve the status of Approved Mental Health Professional and/or Best Interest Assessor as per the SWLA Career Pathway
- To undertake and achieve the status of Work Based Supervisor and Practice Educator as per the SWLA Career Pathway
