We have an exciting new role with the Integrated Discharge Service for an expert practitioner in complex discharges with sound clinical acumen around the assessment, case management and planning of complex discharges. This is an integrated service across Imperial College Healthcare and Central London Community Healthcare NHS Trust.

The post holder will provisionally be based on the St Mary's site but will be needed to support and cover the other sites as the service requires.

Responsible for the operations of the discharge team, including the discharge hub function and the discharge lounge, this is a fantastic high profile opportunity for anyone with a passion for integrated care and service development. Main duties of the job

In summary, the Clinical Lead will:
  • Provide clinical leadership and responsibility for a caseload of patients.
  • Provide clinical supervision to discharge team leads.
  • Provide operational management of the discharge team and discharge lounge ensuring robust service delivery.
  • Deputise for the discharge leadership team.
  • Lead on operational relationships with community partners and local authority adult social care teams.
  • Use operational and performance data to feed into oversight calls and implement the North West London escalation framework.
  • Foster strong relationships with the general and acute adult inpatient multi-disciplinary teams in order to promote acute flow and the reduction of discharge delays.

Person Specification

Education/ qualifications

Essential

  • BSc/1st degree (health related) or equivalent qualification/training and experience
  • Professional Registration in an appropriate health care discipline (e.g., RN or AHP)
  • Relevant specialist training, experience, and short courses to master's level equivalent.
  • Teaching and assessing or mentorship course.
  • Completion of an appropriate management / leadership programme


Desirable

  • Quality Improvement or Project Management course


Experience

Essential

  • Demonstrable experience of complex discharge case management and problem solving
  • Demonstrable experience of managing a range of professional staff and working in multidisciplinary teams
  • Substantial experience in working across organisational and sector boundaries
  • Teaching experience (informal or formal)
  • Recruitment, retention and performance management experience
  • Experience of working in partnership with voluntary / statutory agencies
  • Experience of implementing service level change


Desirable

  • Experience of leading audits and implementing action plans
