This is a unique opportunity for an experienced Speech and Language Therapist seeking a new challenge in an educational setting with young adults aged 18-25 years old, at Horizons College.

Main duties of the job

To provide a specialist speech and language therapy service to adults with a learning disability and or autism.

To work within a multi-disciplinary team to provide SLT services in the community.

To provide effective communication and joint working with patients and carers

You will be an enthusiastic individual with excellent motivation and communicative skills.

It would be beneficial but not essential to be a clinical educator and be involved in the teaching of other disciplines.

The post is part-time, based on site at Horizons College. There will be flexibility built in to your time, to be part of the wider Specialist Learning Disability service and mainstream health and social care providers. Other than exceptional circumstances, annual leave is to be taken during Horizon College holidays (that is, not during term time), as it equitable with education staff.

Job description Job responsibilities

To provide a specialist speech and language therapy service to adults with a learning disability and or autism.

To work within a multi-disciplinary team to provide SLT services in the community.

To provide effective communication and joint working with patients and carers

To liaise effectively with other services across professional and organisational boundaries

Key result Areas

Manage a caseload of clients with a range of communication and swallowing difficulties, as allocated from the multidisciplinary team.

Assessment and diagnosis of swallowing and communication difficulties.

Implement programmes of treatment, as agreed with service users and carers.

Evaluate treatment programmes according to SLT specific outcome measures and such measures as used within the service.

Provide advice and ongoing support for the management of swallowing and communication difficulties in a variety of settings, including care homes and the persons home.

To act in a consultative capacity within the team and the on behalf of the wider service, in contributing to the management of people with complex needs.

Actively contribute to the strategic development of the service within the wider development strategies of the LDU and AWP.

Provide statistics and other information for use in research projects and quality reporting.

To develop and promote the use of assistive technology in adults with LD/ASD
