We are look for a physiotherapist who will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and facilitate early discharge to improve patient health outcomes. Patients requiring case management in the Rapid Response and Virtual Ward service will be identified by case-finding in the Same Day Emergency Care (SDEC) unit, Accident and Emergency department, NMUH wards and also via Community referrals e.g., GP, Care Homes, Community Nurses, and LAS.
The post holder will be a senior physiotherapist within the NMUH VW Service and therefore act as a clinical leader and role model to staff, offering therapy advice on the care of patients with complex needs as required. They will work with colleagues and managers to support the integration of care pathways and patient care across community and acute settings.
Main duties of the job
Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing therapy needs of patients on the caseload.
Undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies
Utilize an extensive knowledge of evidence-based practice and treatment options to select and deliver a specialised physiotherapy programme to individual patients to prevent unnecessary admission to hospital.
Accept clinical responsibility for a designated caseload of patients
Assess accurately for and prescribe a range of equipment and to be responsible for the safe and competent use of equipment, through teaching and supervision of practice.
Deliver the Physiotherapy element of the team and, where necessary, deliver multi-professional care when competent to do so.
Participate and lead in developing Care Pathways.
Participate in and lead MDT meetings
The post holder will be expected to co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary admissions, facilitating appropriate ones through in-reach case finding in both the hospital setting also via GP referral.
Care, compassion, competence, communication, courage, and commitment will underpin all care delivered as well as Standards for Better Health Care, National Service Frameworks, the long-term conditions agenda, hospital avoidance and care closer to home.

- Provide specialist, high quality, expert physiotherapy assessment and care and support for patients who are suitable for Virtual Ward (VW) care in liaison with the multidisciplinary teams (MDT) through assessment and care planning.

Education and Qualifications
Essential
- H.C.P.C. registration
- Evidence of CPD portfolio of ongoing relevant post - graduate training both theoretical and practical to a high level. This will be accredited at Masters or equivalent.
Desirable
- Member of professional body or interest group.
- Any other relevant training and education.
Experience and knowledge
Essential
- Recent substantial experience at band 6 level/equivalent in relevant area and able to demonstrate appropriate postgraduate experience working across physiotherapy core areas to support the rehabilitation needs of the patient.
- Experience of working as a part of a team.

- Experience of inpatients and multidisciplinary services.
