The community rehabilitation service provides care and support for people with complex mental health difficulties that have impacted on their daily lives.

They explore and provide a wide range of goal-based interventions with people to support their recovery. Individualised programmes of care are developed to enhance independence, build meaningful activities and improved health and wellbeing. There is an emphasis on holding hope for people in their recovery. The programmes are offered in peoples own homes and can involve work in the local communities. The successful candidate will be expected to advocate for the people in their care and promote a recovery orientated focus throughout.

The post requires lone working and will include 1 to 1 sessions and specific group work. The core hours of the post are 09:00 till 17:00 week days., Monday to Friday. There is an expectation for some flexible working between the hours of 08:00 and 20:00 including some evenings, weekends and public holidays if there is a service need.

The service operates in three localities in East Kent, with each team consisting of a combination of community mental health nurses, occupational therapists and support from third sector workers. Collaborative working is essential, with close partnerships with Social care, individual care providers and Housing services. This coupled with the wider health services

Main duties of the job

The Senior Community Psychiatric Nurse is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience.

The Psychiatric Nurse will be professionally accountable to the Executive Director of Nursing, and operationally accountable to the Service Manager

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Person Specification

Training, Qualifications and Registration

Essential

  • Qualified and registered as an RMN with the approval to practice in the UK. Completed and signed off preceptorship.
  • Registration with the NMC
  • Approved Practice Educator


Desirable

  • Further training relevant to the role


Experience

Essential

  • Minimum of 2 years post registration experience
  • Supervision and appraisal junior staff.
  • Experience of working with complex mental health conditions in a rehabilitation setting
  • Experience of individual & group work


Desirable

  • Experience of leading service developments
  • Experience of clinical audit and research


Knowledge and Skills

Essential

  • To have demonstrable advanced skills in Mental Health assessment formulation, risk assessment and care planning
  • Knowledge and expertise in the application of relevant theoretical models, and approaches relevant to client group
  • Understanding of the impact of mental health needs and/or cognitive impairment on function and occupation of clients, family and carers.


Desirable

  • Training and presentation/facilitation skills
