An exciting opportunity has arisen to work with the Community Nursing Service in Reading based at 7-9 Cremyll Road, Reading, RG1 8NQ. You will be working in the role of Community Nursing Sister an important part of the ever-evolving Community Nursing service.
The core hours of the service are 08.30-19.00 Monday- Sunday however the hours are negotiable. You will be responsible for providing care to housebound patients. In your role you will lead and support staff in order to promote a service which provides complex nursing care to patients in their own homes. You will be responsible for all aspects of the leadership, development of junior staff and managing a patient caseload within your team with support of the Team Leads.
Using your skills you will ensure that safe, evidence-based care is delivered to patients, their families and carers in the Community, whilst also being proactive in promoting a positive image of Community Nursing. You will be working closely with the organisation, integrated teams, key stakeholders and partners.

Main duties of the job
- To operate within a Case Management framework and coordinate care for an identified caseload of patients.
- To be responsible for assessing patient needs and to plan, implement and evaluate programmes of Nursing Care.
- To proactively monitor patients following the review dates set out in the care plans.
- To make referrals to other services or professions, request diagnostic tests and instigate treatments and interventions as required
- To assess patients with complex clinical need using clinical examination skills, history taking and evaluation skills
- To manage End of Life Care and work in partnership with health, social and voluntary agencies to ensure patients are treated with dignity and respect and to support their families and carers.
- Directly manage a team, providing leadership, line management responsibilities supporting individual colleagues and a team.

- RGN
- Nurse Prescriber/Extended Prescriber Qualification
- Teaching qualification/or willingness to undertake
- Evidence of on-going professional development
- Minimum of two years recent experience in a community nursing team.
- Primary care experience
