We are looking for a Renal Social Worker who is able:
- To provide a social work service to clients under the care of the renal department.
- To provide a full psychosocial assessment of patients and continuing psychosocial support.
- To liaise with other health and social care professionals both within the Trust and the Community.
- To work as a member of the renal multidisciplinary team (MDT) and to provide appropriate social work input to that team.
Main duties of the job
- Find solutions to problems ie Financial & benefit assessments.
- Psycho- social support.
- Multidisciplinary team Meyerbeer of the Renal team.
- Helping protect vulnerable people renal patients from harm or abuse.
- Supporting renal patients to live independently where appropriate.

- Certificate of qualification in social work (CQSW) or Diploma in Social Work Appropriate Degree 3 years post qualifying experience Current registration with the Health & Care professional council (HCPC)
- Current registration with the Health & Care professional council (HCPC)
- 3 years post qualifying experience

- Hospital or Hospice Experience
- To have or be willing to work towards PQ1 and other professional development courses
