Lincolnshire Community Health Services are seeking a full time District Nurse or Case Manager to join them within the Gainsborough Community Nursing Team on a fixed term basis for 10 months to cover a secondment.

This is an excellent opportunity to gain insight into the role as a development or as an introduction to the organisation.

Main duties of the job

The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. They will need to have extensive communication, interpersonal and clinical skills in order to be successful.

The District Nurse/Case Manager is not only integral to leading on patient care but leading the community nursing team through challenging times and the transformation of services, role modelling excellent behaviours which are in line with our trust values. The District Nurse/Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience.

As this is a community post, you must be able to travel to patients homes and clinic settings. Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential.
