The HUB will have a dedicated Psychical Health Team, for patients on specific medications, as well as completing the general required observations. In addition this team will deliver low level one to one work along with ungoverned group interventions.

The Intervention Team will be staff trained to work with the complex patients that require more specific interventions to meet their mental health needs. As we move away from Care Coordination, and with this new model we envisaged a team that will be therapy focused and highly trained.
