In Somerset Foundation Trust Children and Adolescent Mental Health Services (CAMHS) we are excited to have the opportunity to work with our colleagues in the Primary Care Networks (PCN) in improving access for young people who need our help and support.
The Community Mental Health Hub, is designed to provide young people with expert help from one easy access point, and will be hosted by the CLIC (Chard, Langport and Ilminster) PCN. The main goal for PCNs, as set out by NHS England, is to enable greater provision of proactive, personalised, and coordinated care.
Main duties of the job
This is a new service which creates a unique opportunity for you to help shape mental health services for young people alongside experienced and supportive Primary Care colleagues / practitioners. You'll provide assessments and short-term interventions as well as offering consultation to colleagues andsignposting young people to services that will be able to meet their needs in a timely manner and prevent them being 'bounced' around the systems.
You'll be supported by a clinical lead and be part of a growing network of Primary Care Mental Health workforce. You'll be encouraged to develop professionally and personally via training and development opportunities in the Trust and PCN.
The team link closely with other CAMHS teams, such as the Single Point of Access Team, as well as Voluntary Sector and Schools. This opens the opportunity to work collaboratively whilst putting the young people at the centre of everything we do.
This is a brilliant opportunity for someone with a background in nursing, social work, psychology or occupational therapy who can work autonomously and part of an MDT team and shares our dedication to helping young people get the right support at the right time.
This post covers the Chard, Langport, Ilminster and Crewkerne (CLIC) Primary Care Network (PCN) which comprises of 7 GP surgeries and serves an all-age population of 48,000 people. Our lovely colleagues within the PCN are excited to welcome you!

- To work as part of the Mental Health Hub undertaking direct/indirect assessments and consultations, offering advice, signposting and undertaking the appropriate assessment of risk.
- To support the delivery of a high-quality link worker provision into primary and secondary care. This will include providing consultation and training to staff in a range of agencies in relation to the mental and emotional well-being of children and young people.
- To support the PCN in the delivery of the service, ensuring it meets national guidance and local key performance indicators.
- To provide clinical supervision if appropriate and as required
Person Specification Qualifications Essential
- Core qualification in Nursing, Social Work or Occupational Therapy
- Evidence of continued professional development
- oProfessional knowledge acquired through degree/diploma.
- Post qualification experience in a mental health setting, working with young people with mental illness.
- Experience of working with children and young people who have serious mental illness and their families and carers in the community
- To have ability to manage a case load and experience of acting as care coordinator for clients on a case list
- Able to demonstrate experience of providing mental health assessments
- Experience of managing risk in a community setting
