BAND AND SALARY: Band 7
NUMBER OF HOURS: 1.0 wte (37.5 hours per week)
TEMP/PERM (END DATE IF FIXED TERM): Permanent contract

We work closely with the local community to design and improve services, ensuring what we provide is best suited to the patient, their carers and their other family members. This role is significant in moving towards an integrated care system approach and successful candidates will be trailblazing this new approach.
We are looking to recruit a substantive, full time senior mental health practitioner to work in the Primary Care Network. Whilst based within the locality Integrated Place Based Team you will be assigned to the Primary Care Network's (PCNs) to work autonomously in GP Practice’s to provide mental health leadership and implement clinical processes. In addition to undertaking mental health assessments you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care. Ideal candidates will be qualified to prescribe and support with medication changes.
The Successful Candidate will:
• Have the qualifications and experience as highlighted in the Person Specification.
• Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
• Preferably have some experience of working closely with or within primary care.
• Demonstrate adaptability, excellent communication skills and self-motivation.
• Be able to demonstrate appreciation of service users needs and a track record of improving care
We are committed to a recovery and person centred approach that focuses on helping individuals to identify their strengths and potential to maintain their own mental health and well-being. The successful applicant will implement evidence-based interventions for clients with a range of mental health difficulties. This will include assessment and liaison with other LPFT services and external agencies. As the post will involve liaison communication and team-working skills are essential. You will work in the context of local and national drivers providing advanced clinical care for identification of individual needs which essentially supports diagnosis and treatment options. You will forward or signpost individuals to the most suitable service dependant on your assessment and crucially co-ordinate the patient’s journey across the health, social care system, including voluntary and third sector partners, regardless of care setting.
The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.
If successfully shortlisted you will be invited to a two stage process (on 2 separate dates). The first stage will be a presentation to a multi-disciplinary panel and the second stage will be a formal interview. Dates will be confirmed to candidates once shortlisting has taken place.
