Across Gloucestershire we have had major investment in our Home First and Reablement team and as a result of this we have an opportunity for a Band 6 Occupational Therapist to join our leadership team and be a key part of developing our therapy led service in the South Cotswolds. Our Team is based in Cirencester, and we work closely with the local GP practices in the South Cotswold Primary Care Network, alongside our Adult Social Care colleagues and voluntary organisations in the locality.

In line with the Government hospital discharge and community support policy, and the discharge to assess model, our Home First service is a time limited assessment service for people in their own homes to facilitate rapid discharge from acute and community hospitals.

We also provide Reablement to patients in the community, to prevent hospital admission and to facilitate maximum independence, working on jointly agreed goals, promoting personalised care.

Main duties of the job

As an Occupational Therapist you would be a key part of our team, providing assessment, goal planning and reviews, working alongside Physiotherapists, Nurses, Reablement workers and rehabilitation assistants to enable our patients to gain their maximum level of independence in their own home. The caseload will be varied, ranging from rapid assessment to prevent hospital admission, falls response work and Home First assessment to establish pathways of care following hospital admission..

Person Specification Educational Essential
  • 1.Educated to degree standard in Occupational Therapy.
  • Relevant post-graduate qualifications .
Clinical Essential
  • Consolidated experience of recent and relevant Occupational Therapy practice; including experience of managing long term conditions/complex patients/rehabilitation.
  • Evidence of ability to effectively manage workload, including prioritisation skills and delegation
Managerial Essential
  • . Evidence of relevant leadership experience; to be competent supporting and managing more junior staff and students.
  • Evidence of effective communication skills; including liaison with other health care professionals and external agencies, as well as MDT working.
  • . Evidence of initiative, autonomous working, good time management and organisational skills. .
  • Relevant post-graduate qualifications .
