The Integrated Discharge Liaison Therapist / Nurse is expected to support the development and clinical effectiveness of the Integrated Discharge Team and support integrated system working in order to facilitate safe and timely discharge. The postholder will be link with the wards within North Middlesex Hospital supporting ward teams to identify patients medically optimised for discharge and support with discharge planning and require paperwork. This role is ideal for a nurse / AHP with varied clinical experience in acute and community settings and passionate to ensure patients receive the right care in the right place following hospital admission.

The post holder will have the responsibility to work as an integrated member of a system-wide Health and Social care multidisciplinary team to ensure the hub is meeting the patient and service needs, providing expert advice to ensure effective delivery of the service in accordance with the service specification.

Main duties of the job

  • To work closely with the ward teams at North Middlesex Hospital and the system partners within the integrated discharge team to support timely discharge from hospital into the community
  • To engage and support ward teams with discharge planning, paperwork identify safe discharge destinations utilising and maximising on community services as support
  • To maintain oversight of national and local policies, relating to Medically Optimised for Discharge Transfers, and ensure that these are understood, and reflected in Trust policies and procedures
  • To support local services and teams in managing complex discharges and application of the Choice Policy.
  • To attend and support on system calls to manage discharge and patient flow
  • To develop relationships with staff within the integrated discharge team and ward team


  • About us

    The integrated discharge team (IDT) is part of the Enfield Community Services Division within BEH and are based at the North Middlesex Hospital site. The team are a newly formed team during the COVID19 pandemic and focus on ensuring patients are discharge to the appropriate destination with the right level of support and intervention from community services and work across the NCL system and with system partners in the CCG, neighbouring IDTs, social services, third sector and other community teams.

    Person Specification Education and qualifications Essential
    • Bachelors Degree
    • HCPC / NMC registered
    • Knowledge of national best practice in system working and discharge
    • Awareness of National Policy and drivers in Discharge Planning and Patient Flow across the whole health and social care system.
    • Knowledge and understanding of Clinical Governance and risk management systems.
    Desirable
    • -Knowledge and understanding of the operation of a major acute hospital, community trusts and across social care sector.
    Skills and Abilities Essential
    • Ability to motivate staff in different settings
    • Ability to demonstrate awareness of need for effective team work
    • Excellent time management
    • Computer literate (with ability to generate reports)
    • Effective people management skills
    • Good verbal and written communication skills
    • Able to multi task, prioritise and continue to function to a high standard when under pressure

    • Understanding and insight into health policy related to discharge - choice policy, discharge to assess pathways
    • Varied clinical experience - acute and community
    • Knowledge of current challenges in the NHS
