- To improve the lives of people with Sever Mental Illness (SMI) and support primary care settings to provide treatments and interventions in a way which helps people with SMI to access physical health provision.
- To provide continuing responsibility and accountability in the delivery of a primacy care mental health Service and working closely with the Primary Care Network, the primary and Community Mental health wider team members, mental health services stakeholders.
- To provide a direct specialist mental health psychosocial intervention to adult and older adult service users with severe mental health needs, to help them reduce the negative symptoms of mental ill health.
- To develop and maintain effective working relations within the multidisciplinary team representing your professional background as a mental health practitioner. Each month there is dedicated professional development time delivered by experienced clinical leads.
- To provide specialist psychosocial assessment and advice and consultation on clients' mental health and wellbeing with colleagues from Primary care settings and within your multidisciplinary team. You will to work autonomously within professional/voluntary sector colleagues. You will work to the services' policies and procedures which is delivered in partnership with Primary Care Sheffield and Sheffield Mind.
Main duties of the job
- Able to work independently in a number of environments as an autonomous practitioner.
- Provide a comprehensive specialist mental health assessment and identification of a referred patients mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patients
- Regularly review caseload management with their clinical supervisor
- Carry out procedures in accordance with best practice and within own skill set or under supervision of others.
- Maintain accurate records about the care and treatment provided.
- Complete with patients outcome measures and encourage the completion of patient experience measures.
- Review and evaluate professional standards in conjunction with the quality Team
- Ensure that the developing requirements of the Mental Health Nursing/ social work/ Occupational therapy profession are understood by the wider team
- Ensure that clinical, supportive and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Primary and community mental health service policies.
- Adhere to confidentiality principles and maintain the rights of individual patients.
About us
The Primacy and Community Mental Health Service is delivered in a mix of on site and at home working (virtual appointment). The mental health practitioner role is expected to see 26 appointments each week with 5 new patients each week. The appointments are 45 mins long. You will provide treatment to patients which is planned is no more than 16 sessions in length. Patients are to be seen either weekly or Bi weekly, with future booked appointments booked well in advance.
The mental health practitioner will need to conduct robust and comprehensive Biopsychosocial assessment of need for the individuals who present with a wide range of needs. This will then inform the treatment plan.
The community mental health nurse/social worker/Occupational Therapist will have knowledge of working with individuals who experience suicide, self-harm, acute mental illness presentations and Dementia / confusion /delirium.

Person Specification
Training and Qualifications
Essential
- Evidence of continuous professional development in mental health nursing /social work/ Occupational therapy
- Evidence of higher practice skills in mental health care across the age range
- RNMH with current registration with NMC /Social Work Qualification/ occupational therapy
Desirable
- Post reg study to degree or post grad diploma level.
- Teaching/Assessing qualification or equivalent
- To have experience in delivery of training to nurses and other disciplines.
