The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day-to-day basis. This would include the holistic assessment of patient care needs and the development, implementation, and evaluation of evidence-based care.

The post holder will be autonomous in their clinical practice whileacknowledging their limitations and actively seek advice as necessary. We are able to support two newly qualified students within our team with a named working mentor, PTHB preceptor ship program and a very supportive team.

Main duties of the job

The post holder's will enhance their skills and professional portfolios working across the two community settings, visiting patients in their own home and facilitating and support nursing care on the Health & Social Care In-reach unit. Any successful applicants will be supported by the clinical lead. Your personal development plan will be tailored to your individual need based on your experience and transferable skills

We are looking for motivated, enthusiastic, individuals to join the Builth Wells and Rhayader District Nursing Team. We welcome new registrants and experienced staff who share our values, who want to consolidate new skills and learning within a fully supportive and individualized preceptorship program. We need somebody who is caring, sympathetic and able to learn new skills with excellent communications skills and an attitude that ensures the highest standard of care for our patients.

Ability to travel between sites in a timely manner is essential.

The ability to speak Welsh is desirable for this post English and/or Welsh applicants are equally welcome to apply.

  • Palliative care to include syringe driver management
  • Chronic Disease management
  • Wound management
  • Catheterisation - suprapubic, female and male
  • Venepuncture

  • Bowel management
